If you want to show name and surname field, you have to put the respective parameters in the eNews extended plugin. Here I am talking about Mailchimp form set up. You have to get the below information first.

Form Action: From your mailchimp dashboard get the mailchimp sign up form code and get the code mentioned under < form action = ” Copy the Text of this area ”

Email Field: EMAIL

First Name Field: FNAME

Last Name Field: LNAME

Put an header to show as form heading. You can also write some text before and after the form fields as well as per your need.
